        第三部分绩效管理如何应对基于DRG/DIP的医院高质量发展战略

        （一）创新“战略－预算－绩效”绩效管理新模式是根本

        必须建立“战略－预算－绩效”三位一体的绩效管理新模式，因为高质量战略决定医院优化业绩的方式，财务预算决定医院的业绩优化效果，而绩效决定了医院业绩优化的方式及效果的实现。只有这样，才能从根本上系统解决医保控费背景下的战略应对、财务风险及绩效对策。

        （二）“战略－预算－绩效”三位一体的绩效管理新模式

        1.一个目标：医院高质量发展与财务预算目标。

        要将医院基于DRG/DIP的高质量发展重点目标逐一与临床科室沟通，形成相应的科室战略目标、应对计划、资源配置及收支预算，并完善医院层面的重点战略目标、计划及三大预算；要根据合理的财务收支预算，合理确定人工成本预算和绩效预算。

        具体要突出四大战略目标：

        （1）就诊量是前提；

        （2）提升有效收入是关键；

        （3）医保控费及成本控制是基础；

        （4）提升手术微创占比及CMI值是核心。

        2.八大步骤：

        （1）业务科室绩效目标分解

        要将医保控费背景下的医院高质量发展目标，特别是四大重点目标合理分解到业务科室。

        （2）业务科室定岗定编

        要基于科室业务目标、学科建设规划等，结合行业大数据确定定岗定编，这是绩效分配的基础和人工成本控制的基础。

        （3）岗位价值系数设计

        科学测评确定岗位价值系数，作为绩效预算的基础。

        （4）科室价值系数评价

        根据人均有效收入、就诊量、手术微创和CMI三大目标确定科室系数，导向医保控费下医院高质量发展的要求，公平合理地确定科室绩效差别。

        （5）业务科室一级、二级绩效考核模式设计

        根据医保控费下高质量发展的应对，设计“优结构、提质增效”的三大指标体系；并基于岗位价值、特别是业务结构、质量与效率进行合理二级分配。

        （6）行政后勤一级、二级绩效考核模式设计

        合理确定职能部门、岗位及个人系数，合理进行定岗定编，根据医保控费下高质量发展的应对，构建职能部门“优结构、提质增效”的三大指标体系、计划执行率和360服务协作考核体系；构建职能部门二级分配体系。

        （7）核心人才季度/年度考核方式

        根据医保控费下高质量发展的应对，构建核心人才“优化结构、提质增效”的三大战略指标体系、计划执行率和核心人才素质考核体系。

        （8）绩效实施配套措施

        包括组织、数据、实施配套措施等。
