        德州市中医院是集医疗急救、康复保健、科研教学为一体的三级甲等中医医院，医院现为滨州医学院附属医院、北京东直门医院协作医院、国家中医住院医师规范化培训基地、山东省中医药预防保健服务中心、山东省中医类别执业医师实践技能考试基地、山东省“西医学习中医”培训基地、山东省中医药文化建设示范单位、德州市工伤康复定点医院、德州市脑瘫儿童康复定点医院。
        
        医院分东、西两个院区，总占地面积115.58亩，总建筑面积7.9万平方米，编制床位498张，开放床位650张。中医药人才储备：有国家级、省级、市级名老中医药专家传承工作室指导教师8人，国家高层次优秀中医临床人才2人、省高层次优秀中医临床人才2人、省名中医药专家8人、省五级师承指导教师6人，省中医技术骨干5人。
        
        【项目背景】
        
        在公立医院从规模发展转向高质量发展的背景下，受德州市中医院委托，2022年底九鼎医管启动德州市中医院基于高质量发展战略的绩效管理咨询项目，项目命名为“杏林计划”。该项目通过对医院进行调研诊断，综合分析医院外部发展环境和内部资源条件，给出医院战略定位建议，以重视国考等相关政策要求为前提，明确医院功能定位，促进医院综合实力提升；并结合当地医疗政策，综合分析中医市场需求，进行专科规划建议，提升人才技术水平；以适应DIP医保付费政策为方向，分析医院及科室运营提升策略；最终将医院的战略定位、学科建设以及运营战略三大战略决策与绩效管理相结合，充分调动员工积极性，将高质量发展新形势下做大蛋糕与分蛋糕相结合，以提升医院业绩与员工待遇为目的，建设符合医院未来发展需要的SDO-P战略绩效管理体系，最终实现医院品牌力的提升和业绩的增长，促进医院的高质量发展！
        
        【项目历程】
        
        2022年12月底，九鼎项目组进驻医院开展系统调研工作，开展医院内部人员一对一访谈36人次；发放员工问卷调查200份、患者问卷调查100份；开展岗位评价会议1场，并收集战略、学科、运营、绩效等相关资料；
        
        2023年4月，九鼎项目组提交第一期咨询方案，包括《德州市中医院战略绩效管理诊断与高质量发展建议报告》、《德州市中医院业务科室基于高质量发展的战略绩效管理手册》、《德州市中医院基于高质量发展的组织与岗位管理方案》等，并就方案分别与医院领导、主要职能部门进行了深入研讨和发布。
        
        2023年5月，九鼎项目组赴医院进行医院绩效管理方案讨论及业务科室一对一沟通工作，与临床科室科主任、护士长分别进行沟通共计37场，就绩效方案设计的指导思想、逻辑结构、具体绩效考核方式、各科室新方案模拟测算结果与各科室进行了详细沟通，对各科室反馈的意见和建议进行全面收集、分析、研究。
        
        2023年7月，九鼎项目组赴医院进行7月业务科室绩效方案核算结果讨论及职能科室绩效方案及测算结果提交工作，参与职代会宣讲新方案，辅导医院进入新方案试运行。
        
        2023年8月-9月，九鼎项目组辅导医院方案运行，推进职能科室绩效管理方案落地。
        
        2023年10月至今，九鼎项目组与医院保持持续沟通，为方案实施进行顾问指导。

        【项目亮点总结】
        
        守正强基——基于高质量发展战略背景，本项目首先明确医院发展战略定位，从区域定位、院区定位、中西医业务定位、品牌定位等角度确定医院发展愿景与目标，体现传承与创新，围绕“初步建设成为全国知名，山东一流的三甲中医区域医疗中心”的战略目标确定医院的各项年度目标。
        
        其次，高度重视国考指标管理与考核，结合医院国考指标实际情况，筛选出七大重点管控指标，包括“门诊中药处方比例、门诊散装中药饮片和小包装中药饮片处方比例、门诊患者中药饮片使用率、门诊患者使用中医非药物疗法比例、出院患者中药饮片使用率、出院患者使用中医非药物疗法比例、以中医为主治疗的出院患者比例”等，参考各项指标的省内最大值、省内平均值、省内中位值、省内最小值等确定各项指标的院级目标，再结合各科室半年度的数据，结合各科平均值、最大值、最小值分解形成各科室的考核目标值，将国考指标纳入月度常态管理与考核沟通。
        
        同时，基于市场潜力、科室竞争力，综合考虑三级中医医院等级评审和医院发展战略需求明确医院整体学科规划，并将学科规划与建设的相关指标如如国家级、省级、市级重点（建设）学科（专科）数量；亚专业细分、分科、多学科中心数量；新技术新项目数量；病危病重出院患者人次及占比；三四级手术例数；CMI值；低风险组死亡率、中低风险组死亡率、甲级病案率等；科研项目的立项数、教学指标、各学科人力资源规划、学科相关资源整合等体现在绩效管理方案中，通过考核促进医院人才与技术提升。
        
        创新发展——公立医院高质量发展的核心来自于创新，本项目方案设计中高度重视对于创新的激励。
        
        一是以鼓励技术创新为重点，适应DIP医保付费的新趋势。
        
        医院以鼓励技术进步为核心，首先体现CMI值的激励，在绩效方案设计中，通过CMI值与各科室的人均工作量进行关联，应用于科室医、护团队系数的设计，合理确定各科室医、护团队的价值排序；加大手术操作性业务、中医适宜技术特色业务考核激励：特别鼓励三四级手术、微创、介入、内镜等技术含量高的业务；鼓励DIP付费下的各种协作业务，如临床与临床协作、临床与ICU协作、临床与康复协作、临床与医技协作、临床医生与护理协作等业务激励；鼓励MDT、会诊业务；未来在完善数据的基础上，将进一步建立病种结构优化考核激励：根据CMI值和病种分值划分重点激励病种和一般激励病种，赋予不同分值与医疗团队的绩效工资预算挂钩，特别鼓励分值高的病种；鼓励中医优势病种出院人次；
        
        二是以鼓励管理创新为保障，促进医院管理与业务齐头并进。
        
        为适应医院新时期高质量发展要求及三甲中医院复审、三级中医院绩效考核等对医院职能管理提出的更高要求，本项目通过实施职能部门绩效管理改革，建立职能部门分档、职能岗位分级、基于岗位价值与个体价值相结合的个人绩效系数设计，充分体现体现多劳多得、优劳优得；绩效分配向对个人能力、综合素质要求高的、贡献价值高的岗位倾斜的分配导向，通过考核强化职能部门服务临床业务一线；通过绩效管理为员工持续赋能，激励员工积极主动追求成长，持续提升自己的综合素质与能力。
