  全省在医疗卫生领域累计投入资金达441亿元合理增加薪酬总量，全省公立医院人员支出占比57.45%

  会上，青海省卫生健康委党组书记、主任任延明介绍，青海省学习推广三明医改经验，推动医疗、医保、医药协同发展和治理的实践与成效。

  一、高位引领，推动医改攻坚破局

  “十四五”以来，全省在医疗卫生领域累计投入资金达441亿元，围绕完善医疗卫生服务体系、公立医院高质量发展、深化医疗保障制度改革等方面出台一系列政策文件，为改革任务顺利实施提供坚实保障。

  二、完善体系，优化医疗服务供给

  一是围绕“大病不出省”，加快优质医疗资源扩容。截至目前，全省公立医院出院患者三四级手术比例52.54%，疑难重症诊治水平逐步提升。

  二是围绕“一般病在市县解决”，提升医疗服务能力。全省共组建46个紧密型县域医共体，精准实施“组团式”援青、“青南环湖”支医和巡回医疗等帮扶机制。

  三是围绕“头疼脑热在乡村解决”，夯实基层基础。65.91%的乡镇卫生院和社区卫生服务中心达到国家服务能力标准。全面实施大学生乡村医生专项计划，招录的537名大学生乡村医生全部纳入编制，提升村医队伍素质。截至目前，全省县域内住院量占比为54.02%，基层医疗机构诊疗量占比为45.66%，基层医疗服务能力稳步提高。

  三、公益导向，赋能医院提质增效

  一是推进公立医院高质量发展。西宁市获批国家公立医院改革与高质量发展示范项目，中央财政给予5亿元资金支持。

  二是深化人事薪酬制度改革。落实“两个允许”，实行分级分类调控办法，优化完善绩效工资政策，合理增加薪酬总量，全省公立医院人员支出占比57.45%，有效调动医务人员积极性。出台完善疾控机构绩效工资政策，逐步缩小公共卫生机构与医疗机构收入差距。推进人事制度改革，调整提高医疗卫生机构专业技术岗位结构比例，实行基层职称评聘“双定向”政策。

  三是动态调整医疗服务价格。“十四五”以来，全省累计动态调整医疗服务价格3675项，涉及调价金额12.61亿元。

  四是开展便民惠民服务。推进省内检查检验结果互认，与陕西等8省区建立省际检验结果互认机制，全省累计互认项目171万次，为患者节约费用4550万元。在13家医疗机构开展免陪照护服务试点，已服务患者2058人次。

  五是推动中（藏）医药传承创新发展。“十四五”以来，累计建设国家中医优势专科16个、省县共建中（藏）医重点专科32个，县级中（藏）医医院建设“两专科一中心”30个。

  四、协同发力，推进医保医药改革

  一是完善医保领域改革。率先实现城乡居民医保省级统筹，实行职工基本医疗保险门诊共济保障，将普通门诊医疗费用纳入统筹基金报销，个人账户使用范围扩大至职工近亲属。率先制定首部藏（蒙）医医疗机构制剂医保目录。DRG/DIP付费方式实现统筹区域、符合条件的医疗机构全覆盖，全省医疗次均费用下降1100余元、平均住院床日下降0.8天，将项痹等6个病种纳入中（藏蒙）医日间病房医保结算范围，群众负担持续减轻。

  二是强化药品供应保障。建立以基本药物为主导的“1+X”用药模式，以省为单位统一医共体药品目录，收录药品1140种。制定儿童常见病多发病药品配备使用指导目录，保障儿童临床用药安全。常态化开展药品耗材集中带量采购，累计集采药品1202种，耗材127类，采购价格平均降幅55%，有效减轻患者用药负担。

  青海省在推进医疗卫生行业人事薪酬改革方面，主要做了哪些工作？

  第一、深化人事制度改革，进一步优化用人机制

  我们积极出台相关政策，优化基层人才招聘办法，不断拓宽人才引进渠道，加强医疗卫生人才队伍建设。

  一是创新优化引才方式。对高层次急需紧缺人才，可不受岗位限制，采取设置特设岗位的方式先引进后消化，有效提高引才效率；同时，适当放宽基层医疗卫生机构招聘条件，通过约定不超过5年服务期限，帮助基层卫生机构留人用人；积极配合省卫生健康委完成大学生乡村医生专项招聘计划，充实和稳定基层医疗卫生人才队伍。“十四五”期间，全省发布医疗卫生事业单位招聘引才岗位4400余个。

  二是分类设置岗位结构比例。分类优化设置公立医院、疾病预防控制等医疗卫生事业单位专业技术岗位结构比例。支持医疗卫生事业单位在核定的岗位结构比例内，自主设置岗位方案、聘用条件、考核办法，健全完善以品德、能力和业绩为依据，以工作实绩为标准的岗位竞聘机制。坚持向基层和艰苦地区倾斜的原则，积极推进基层职称“定向评价、定向使用”制度，并单独核定“双定向”高、中级岗位比例，稳定了基层医疗卫生人才队伍。

  三是建立岗位动态管理机制。对承担医学院校教学任务重、认定为国家级住院医师规范化培训基地、承担国家级重大科研攻关任务等医疗卫生机构，在岗位结构比例方面给予适当倾斜支持，加强重点科室、团队建设以及优秀青年人才的培养。对引进和培养的国家级高层次人才、延退的高级专家，所聘岗位可按特设岗位核定并单列管理，发挥岗位资源最大效益。

  第二、深化职称制度改革，进一步完善评价机制

  我们不断健全职称评审制度，鼓励支持医疗卫生领域人才、智力向基层流动，提升基层医疗卫生人员服务水平。

  一是引导人才到基层服务。落实国家关于卫生领域专业技术人才晋升高级职称必须有基层服务经历的要求，明确市（州）级及以上医疗机构专技人员申报副高职称时，须到县级医疗卫生机构或乡镇卫生院累计服务1年以上，促进基层卫生事业发展。

  二是鼓励人才在基层建功立业。首次将医疗卫生领域纳入省内六州基层专业技术人才“定向评价、定向使用”评聘管理工作，并对乡镇基层工作时间长、待聘时间长、任职时间长和业绩突出的专技人员在职称评聘条件上给予了倾斜。截至目前，全省基层医疗卫生单位共有1624名专业技术人员通过“双定向”及单独划线倾斜政策取得相应职称。

  三是加强医疗卫生人才培养。近5年来卫生领域共选拔产生享受政府特殊津贴专家11人、“青海学者”3人、“青海省优秀专业技术人才”5人、“青海省优秀专家”13人。自2022年以来，我们遴选卫生领域55名中级及以上专业技术人员（占我省特培计划培养总人数的34%）到北京大学医学部、北京协和医院、四川大学华西医院等医院培训，部分学员学成返岗后，现已成长为单位领导或科室负责人。

  第三、深化薪酬制度改革，进一步健全激励机制

  我们持续完善医疗卫生机构薪酬制度体系，着力发挥收入分配的激励保障作用，有效调动公立医院和广大医务人员的主动性、创造性。

  一是落实事业单位收入分配政策。组织实施调整基本工资标准和晋升薪级工资工作，提高艰苦边远地区津贴和高海拔地区折算工龄补贴标准，全面兑现年度考核奖，稳步提高了全省医务人员工资待遇。

  二是持续推进公立医院薪酬制度改革。实行分级分类的绩效工资调控水平，建立与考核挂钩的绩效工资总量核定办法，实施一次性绩效工资总量核增机制，为公立医院通过提高医疗服务质量增加医务人员收入给予了充分的政策空间。同时，落实内部分配自主权。允许医疗卫生机构结合单位实际，自主制定绩效工资内部分配办法，通过搞活内部分配，重点向一线岗位和做出突出贡献的人员倾斜，充分调动各类人员的积极性。

  三是加大对基层的倾斜力度。进一步完善基层医疗卫生机构绩效工资政策，明确乡镇卫生院可参照公立医院的办法适当核增绩效工资总量，有效稳定了基层医疗卫生人才队伍。

  青海省在提升医疗服务能力方面取得了什么成效？

  一是不断优化医疗资源布局国家区域医疗中心计划年内投入运营，覆盖普通外科、呼吸内科、急诊ICU等共11个住院科室，拟开放床位300张。开展神经外科、神经内科、心血管内科、大血管心脏外科技术平移，已平移新技术77项，填补我省空白45项，达到国内先进水平13项。

  二是全面推进紧密型县域医共体建设组建46个紧密型县域医共体，覆盖62家县级医院、389家基层医疗卫生机构和46家专业公共卫生机构，累计建成22个县医学影像中心和心电诊断中心、15个医学检验中心、8个医共体中心药房，提升县域医疗服务水平。

  三是持续加强本土卫生人才培养国家实施医疗人才“组团式”援青机制，对口支援青海的6省市选派高水平医疗专家280余名，建立青南环湖支医工作机制，省上每年安排283名医务人员下沉青南环湖地区6州医疗机构，开展年复一年的对口帮扶，帮助6个州的州、县级医院开展临床新技术、培养当地人才，加快提升6州危重症诊疗能力。

  四是推进远程医疗信息化建设建成省远程医疗和远程教育平台，以远程影像、远程心电、远程检验、远程会诊为重点，由省级12家三级医院提供技术支撑，覆盖省、市州、县、乡四级医疗机构。全省共有579家医疗机构接入平台，依托平台已累计开展远程会诊2813次、双向转诊8076例，远程影像诊断39080例、远程心电诊断5093例。
